Life Care Planning is a new approach to dealing with issues created by long life and disability. It involves an inter-disciplinary team that works together to identify present and potential future care needs, locate appropriate care and ensure high-quality care.
WHO IS LIFE CARE PLAN FOR?
A Life Care Plan is for individuals dealing with the effects of aging, disability, or chronic illness such as Alzheimer's, stroke or dementia, or Parkinson's disease. While typical life care plan members are elderly, it is appropriate for any individual who is developmentally, emotionally, or cognitively disabled.
AT WHAT POINT SHOULD THE LIFE CARE PLANNING BE DEVELOPED?
Most people create a Life Care Plan immediately after a triggering event such as stroke or diagnosis of Alzheimer's or cancer. However, pre-crisis planning is ideal for families because you can immediately tap into the resources available and increase the possibility that your elder can remain in their home.
WILL THE LIFE CARE PLAN SAVE OUR FAMILY MONEY?
Many of the services in a Life Care Plan can minimize the financial burden created by the high cost of care.
